Your Rights
You have the right to:
Get a copy of your paper or electronic medical record.
Ask us to correct your medical record if you believe it is incorrect or incomplete.
Request confidential communications (e.g., asking us to contact you at a different location).
Ask us to limit what we use or share.
Get a list (accounting) of those with whom we’ve shared your information.
Get a copy of this privacy notice at any time.
Choose someone to act for you (e.g., medical power of attorney).
File a complaint if you believe your privacy rights have been violated.
Your Choices
You have the right to make choices in the way we use and share your information in the following situations:
Sharing information with your family, close friends, or others involved in your care.
In disaster relief efforts.
Including your information in a facility directory (if applicable).
Contacting you for fundraising efforts.
If you have a clear preference for how we share your information in these situations, let us know. Tell us what you want us to do, and we will follow your instructions.
In the event you are unable to express your preferences, we may share your information when necessary for your care or for your safety.
Uses and Disclosures
We may use and share your information for the following purposes:

We can use your health information and share it with other professionals who are treating you.
Example: A doctor treating you for an injury asks another doctor about your overall health condition.

We can use and share your health information to run our pharmacy, improve your care, and contact you when necessary.

Billing and Payment
We can use and share your health information to bill and get payment from health plans or other entities.
Example: We give information about you to your health insurance plan so it will pay for your prescriptions.

Uses That Require Your Authorization

For the sale of your information,
For sharing psychotherapy notes,
unless you give us written authorization.
If you give us permission, you may revoke it at any time.
Your Rights in Detail
Get a Copy of Your Medical Record
You can ask to see or get an electronic or paper copy of your medical record. We will provide a copy within 30 days. A reasonable, cost-based fee may apply.
Ask Us to Correct Your Record
If you think your record is incorrect, you can request a correction. We may say “no” to your request, but we will tell you why in writing within 60 days.
Request Confidential Communications
You can ask us to contact you in a specific way (for example, home or office phone) or send mail to a different address. We will agree to all reasonable requests.
Ask Us to Limit What We Use or Share
You can ask us not to use or share certain health information. We are not required to agree, but if you pay for a service out-of-pocket in full, we will not share that information with your insurer unless required by law.
Get a List of Those We’ve Shared Your Information With
You can request an accounting of disclosures (up to six years back). The first request is free. Additional requests may have a fee.
